Parent Referral

This referral form is to help us better understand your adult child. We desire that your answers help us to determine whether LEGACY is a good fit for them but, more importantly, to help us better understand how we can best mentor and shepherd them if they attend our school. Therefore, we appreciate your honesty as you answer these questions. LEGACY will keep your referral confidential.

For applicants that are of age and no longer financially dependent on their parents, you may choose to have an additional Character Referral form submitted in place of the Parent Referral.
